Improving Transitions in Care -Tools for Individuals Working in Healthcare

As a health care provider, organizational leader,  and/or a policymaker,

why should addressing the ALC issue be a priority?

Here are some fast facts:

Although hospitals in Ontario have guidelines to manage and care for ALC patients, it is unclear how these guidelines are being operationalized in practice and whether they are having an impact on patient care. 

 

Many ALC patients and caregivers still experience significant gaps in care, such as poor communication and a lack of physical activation and psycho-social support

The infographic, briefing note, ALC intervention toolkit and communication tool (below) were put together by our council to highlight the extensive impacts of the ALC issue on our healthcare system and patient outcomes, and propose patient-centered solutions to the problem.
