The Alternate Level of Care Patient and Caregiver Advisory Council, also known as Better Care Journey, has co-designed tools to tackle issues surrounding transitions in care and delayed discharge. Whether you are a policy-maker or decision-maker, organizational leader, educator, researcher, part of an advisory council, or a patient/caregiver, you can browse these tools and join us in our mission to improve the experiences of those impacted by poor transitions in care. 

What is Alternate Level of Care?

PATIENTS PERSPECTIVE

Imagine you are a patient who is admitted to the hospital. Over the course of a few days or months, you receive  care based on your health needs, and you are now ready to be discharged. Although you may still require care, you no longer require the intensity of care delivered in a hospital setting. Unfortunately, you are unable to leave the hospital for an array of reasons.

CAREGIVERS PERSPECTIVE

The Better Care Journey Story

As noted above, the delayed transitions in care that ALC patients experience is a stressful time for everyone involved, including caregivers and providers. These delays negatively impact the quality of care ALC patients receive and their outcomes, and cause hospitals to operate beyond 100% capacity with patients in hallways as they wait to be admitted or discharged. To help address issues surrounding ALC, a group of dedicated ALC patients, caregivers, partnered with a research lead to form the Alternate Level of Care Patient and Caregiver Advisory Council, also known as Better Care Journey.
